Welcome to Hyde Park Schools' Health Services! At Hyde Park, the health and safety of your children is our highest concern. This page provides much of the information and related forms students need. However, our Health Services staff is happy to answer any questions you might have.

Please make sure your child's immunizations are up to date. All records must be current by the first day of school. If a student is not current, he or she will NOT be permitted to attend school. Please upload  a copy of all the updated records to your child's SNAP Health Portal.

Required Screenings

Vision
The state requires vision screening on all NEW students and those in 4K, Kindergarten, 1st, 3rd, 5th, and 7th grade.

Hearing
The state requires all NEW students and those in 4K, Kindergarten, 1st, 3rd, 5th, and 7th have hearing screenings.

Scoliosis
The state requires spinal screenings for Girls age 10 (or fall semester of 5th grade and age 12 (or fall semester of 7th grade) and Boys age 13 or 14 (or fall semester of 8th grade).

The State Health Department requires that all schools report these test results each year. By keeping your child's health record at school up to date, it allows Hyde Park to be in compliance with the state and have a better health history of your child. If there are any questions about the tests or your child's record, please call the nurse's office.

Illness Policy

Illness at school is evaluated by the school health staff on duty. A student with the following symptoms will be sent home from school:

Fever of
100.0 F Orally
Uncontrolled
Diarrhea
Vomiting
Suspected contagious condition / disease

A student must be free of fever (<100.0) for 24 hours without the use of fever suppressing medications to be readmitted to school. In most cases, when prescribed an antibiotic, students must be on the antibiotic for 24 hours before returning to school.

In the event that your child has a health emergency at school, and a parent cannot be reached, he/she will be taken to the nearest emergency facility.

Automated external defibrillators (AEDs) are located on all HP campuses.
